Get ready for your visit.

A little preparation makes your first appointment calmer and more productive. Here is everything you need to arrive ready, finish your paperwork in advance, and know exactly what to expect.

Before You Arrive

Bring these six things.

Having these on hand lets your physician spend the visit on you, not on paperwork.

  • Insurance card & photo IDSo we can verify your benefits and check you in quickly.
  • Your medication listPrescriptions, over-the-counter medicines, and supplements, with doses.
  • Recent labs & imagingAny bloodwork, urine tests, or scans from the past year, if you have them.
  • A referral, if your plan requires oneMany plans need a referral from your primary care physician. We are glad to help coordinate it.
  • Your questions & symptomsWrite down what you want to ask and anything you have noticed. Nothing is too small.
  • Completed new patient formsFill out the forms below ahead of time to save yourself time at check-in.

Completed forms can be emailed to sbryson@napc.care or brought to your visit. For anything containing sensitive medical details, feel free to bring it in person rather than emailing.

What to Expect

Your first visit, step by step.

STEP 01

Check in

Arrive about 15 minutes early. We verify your insurance and finalize any remaining paperwork.

STEP 02

Listen

Your physician reviews your records and hears your concerns in your own words.

STEP 03

Investigate

Targeted labs or imaging where indicated, explained clearly and never ordered reflexively.

STEP 04

Plan

You leave with a written plan, clear next steps, and a direct way to reach the team.

Insurance, referrals & getting here.

We participate with most major commercial insurers, Medicare, and Medicaid plans serving the CSRA. Call ahead and we will verify your benefits before your first visit, so there are no surprises.
